Financial Options
The fee for your orthodontic treatment is based on the severity of the problem and the anticipated length of time to correct it. If treatment is prescribed by Dr. Kuppers, your financial obligation will be presented in the form of a contract between you (the patient or parent) and our office.
Fees are typically broken down into an initial payment, to be paid when treatment begins, with the remaining balance divided into equal monthly payments. The monthly fees are not based on the number of appointments per month or year, but are instead a convenient method of budgeting the fee for treatment over a period of time.
Our fee includes every aspect of orthodontic treatment provided in our office including the diagnostic records, x-rays, appliances, braces, lab fees, regular and emergency visits, the initial set of retainers and monitoring following active treatment. This fee does not include any treatment rendered in any other dental office (dental x-rays, cleanings, check-ups, removal of teeth, replacement of missing teeth, crowns/veneers, cosmetic bonding, whitening, gingival grafts, implants and/or any surgery).
For your convenience we accept cash, personal cheques, debit, Mastercard and Visa. We deliver the finest care at the most reasonable cost to our patients. If you have questions regarding your account, please contact us at 519-884-2556.
Insurance Information
When orthodontic insurance coverage is available, we will complete and provide you with a Standard Orthodontic Information form that you can submit to your insurance company. Most companies do not cover 100% of orthodontic fees, but may provide coverage of a percentage of the orthodontic fee until a maximum has been reached. All orthodontic services performed in this office are billed directly to the patient and/or person(s) responsible for payment. When a payment is made, you will receive a receipt from our office that you can submit to your insurance carrier. You will be reimbursed directly.
Office Hours
We try to stay on schedule to minimize waiting at the time of your appointment. Delays to the schedule can occur and your understanding and patience are very much appreciated. If you have pain or an emergency situation, every attempt will be made to see you that day. Our office hours are Monday – Wednesday from 8:00 a.m.- 4:00 p.m and Thursday from 8:00a.m. - 1:00p.m. |
