First Visit
Please allow 1 hour for each consultation. A minimal fee of $80 per child and $150 per adult (over age 20) will apply. This is payable by cheque, Visa, Mastercard, cash or debit. This consultation fee is covered by many dental insurance plans. Please request an insurance claim form if needed. Upon arrival at our practice you will be greeted by our friendly front office team members, Lesley, Michelle and Cathy. They will welcome you and show you our sign-in station to be utilized at each appointment. This way we know when you arrive so you can be seen promptly. We understand that your time is valuable. A panorex x-ray may be taken if required and one of our records technicians will take digital photos of the face and teeth for Dr. Kuppers. Dr. Kuppers will use this information to determine an orthodontic treatment plan specific for you. Our Treatment Coordinator, Mary Catherine, will introduce you to our practice with a full tour of the facilities. Dr Kuppers will perform a comprehensive exam to identify any potential concerns and make treatment suggestions. Mary Catherine will then explain the results of the exam and the treatment that is being recommended to correct the condition. Be assured that there will be adequate time to have all your questions and concerns addressed at this appointment. Treatment fees will also be discussed and if necessary, insurance forms will be given. Should treatment not be required at this time, Dr Kuppers would be happy to monitor the patients growth and development over a period of time on an observation basis at no additional fee. |
Our objective is to make sure your visit with us will be pleasant and informative. You will receive a Introduction package by mail, which will include a health history and patient consent form, prior to your appointment. Please complete these forms and bring them to the appointment with you.
